VSF Tournament Roster - For teams attending the tournaemnt.  Please fill this form out and have your club registrar verify it and sign it.  This can serve as your tournament roster.

T-shirt Order Form - Used for whole teams to preorder shirts and get a discount.  It also ensures that you will get your size and they do not run out!

Manager Checklist - For team managers to use to ensure that you are properly prepared for registration night and for the event.

Emergency Contact Form - this is required to be turned in at registration and is used for tournament staff to contact you during the event in case of inclement weather or other circumstances.

VYSA Medical Release Form - Here is a generic med release form that can be used for the tournament.  It does not need to be notarized.
